Requesting Services

Break FreeTo request services, simply call or stop by either the Long Beach or Raymond Office. The person you speak with will ask you a few questions, including the reason for requesting services, medical insurance information, level of income, referral source, etc.

A face-to-face intake assessment is then scheduled, with every effort made to see the consumer as soon as possible. In the event of an emergency, a Designated Mental Health Professional can provide crisis support until the consumer can be seen for an intake appointment. The purpose of an intake assessment is to determine qualification for services, including diagnosis of a mental illness, and to discover the strengths and needs in a person’s life they wish to address.

After the intake appointment, a clinician is assigned. This is the professional who will develop a care plan, including goals to be accomplished while in treatment.

Clients Rights
The intake clinician reviews clients rights during the intake appointment and a copy of these are given to the consumer. Further explanation can be requested at any point in treatment.
